National Verifier: USAC-Elected Eligibility Reverification


About National Verifier reverification: The Lifeline National Eligibility Verifier will "soft launch" on December 5, 2017 for the initial set of states, and "hard launch" on March 13, 2018. When the National Verifier launches in a state, the eligibility of all Lifeline subscribers in that state will be reverified. Service providers must provide eligibility documentation for subscribers that did not pass the National Verifier's automated database check. Service providers may elect USAC to conduct the subscriber documentation collection on their behalf. Service providers will still be required to send the eligibility documentation they currently have on file. Complete the form below to elect USAC. Elections are due Wednesday, November 1, 2017.

The initial launch states for the National Verifier are Colorado, Mississippi, Montana, New Mexico, Utah, and Wyoming.
