By registering my child(ren) in Summer Science Camp, I as parent/guardian hereby agree to release, discharge and hold harmless Nazareth College, their officers, agents, and employees of and from all causes, liabilities, damages, claims, or demands whatsoever on account at the Summer Science Camp program, or in the course of activities held in connection with the Summer Science Camp Program.  Additional, I/we authorize the camp and/or Nazareth College to photograph, and/or videotape my/our child in promotion of Summer Science Camp Program and/or Nazareth College.

I further agree to complete and submit the following forms in advance of my/our child(ren) attending camp: Medical, Code of Conduct and Student Release.
